EVALUATION AGREEMENT
It is the goal of the English Language Speech Practice Merenwijk to provide families with thorough speech and language evaluation reports, which can become a part of their personal recorda as they are posted around the world. Evaluations can include standardized and dynamic testing, observations, intervieuws of parents and school personnel, checklists, consultations with previous evaluators, fax and email consultations with previous educators.
A thorough report will be written with recommandations for improved speech and language, which will then be shared with the parents and any appropriate school personnel. It is the strong desire of the English Language Speech Practice that reports not be kept from school, as the goal is to provide recommendations which will help the child become a more successful learner.
The English Language Speech Practice pledges to the English-speaking community that all clients and their families will be threated with utmost respect. Parents are asked to trust the professional judgement of a specialist who has had many years experience.
The following explains the conditions:
Parents will return any release of information, permission to tests, forms, checklists, etc. before the next scheduled session in order for the evaluation to proceed. If not, the sceduled session will be cancelled by the Speech and Language Specialist.
Though the speech and language report will become the property of the parents, the English speech and Language Practice does not want to be a part of reports being kept from school personnel, therfore the speech and language report will be openly shared with those whose task it is to implement recommendations.
Therapy, testing time, any language samples which need to be transcribed and analyzed, report writing, sharing results with parents, and multi-disciplinary meetings at the childs school. Anything else- scoring, analysis of checklists, phone calls, photocopying costs, etc.- is regarded as a task for the specialist.
